I understand that by completing and signing this form I am agreeing to the terms of services selected. I am giving L.J. Lawns permission to be on my property and complete service. I understand that weather and unforeseen circumstances uncontrollable by L.J. Lawns may affect date and/or time of service. Every effort will be made by L.J. Lawns to complete service as scheduled. In the event of a reschedule by L.J. Lawns or weather issue, L.J. Lawns will complete service in a prompt and timely manner. I understand that L.J. Lawns will charge for service at completion of each service. I agree to pay agreed upon price for service. I understand there is a Half Hour minimum charge for every service. If a scheduled service is to be canceled it must be done by phone or email. If an appointment is canceled/rescheduled by client less than 24 hours prior to scheduled time, or unable to be performed due to inability to access property, L.J. Lawns reserves the right to charge full amount for that service.
